This entry form constitutes a formal agreement between the Team Captain and Fivez Ltd. The agreement lasts for a full season, or the remainder of the season if your team join midway through a league season. Any team dropping out of the league or removed for disciplinary reasons will still be liable for the outstanding balance for the remainder of the season.

It is the responsibility of individual team members to arrange their own personal injury insurance. It is recommended that you arrange your own personal injury cover for injuries sustained during play.

I have read and agree to abide by Fivez rules and regulations. By ticking here I understand that both myself and my team are playing at our own risk in any Fivez organised events and Fivez cannot be held responsible for personal injury sustained whilst participating in the leagues. I understand we are entering into a full season and if we pull out of the league before the end of the season we will still be liable for the match fees for the remainder of that season.
