How to join online: 1. Click the following link to complete our 'online entry form' ENTRY FORM 2. Fill out all sections of the entry form and click the 'submit' button. 3. When we have received your entry form we will contact you to inform you of your first match time (we may arrange a trial match to decide which division you will enter or we may put you straight into a league). 4. Fixtures are available on our website (you can also request a copy to be posted if you wish). 5. Simply turn up for your first match and pay your first match fee. There is NO entry fee, affiliation fee or deposit to be paid, you simply start playing straight away. Our staff at the league will give you any extra guidance or help at this point. How to join via email: 1. Email the following information to join@fivez.net: - Your name, address, contact telephone number (home and mobile) and which league you wish to enter. 2. We will send you an entry pack via email. Read through the information and return the Entry Form to us via email after filling out the details required. 3. We will ask you which league you wish to enter and take your name, telephone number and address. 2. We will send you an entry pack in the post. All of our leagues operate on a first-come, first-served basis. The quicker you get your entry form back to us, the more likely it is that you will be allocated a place (leagues are usually restricted to 40 teams).
